Sarcomas originating from the heart and malignant fibrous histiocytoma (MFH) particularly are rarely seen cases. In this article, we present a 65-year-old male case who was initially thought to have thrombus but later was diagnosed with MFH after surgery. The echocardiography performed on the patient admitted to our hospital with exertional dyspnea revealed a heterogeneous mass stretching from the right ventricular outlet tract to inner part of pulmonary artery and to its bifurcation, resulting in severe narrowing. The intraoperative mass resembled an organized thrombus. The mass at the right ventricle and pulmonary artery level, which was initially considered to be a thrombus, was removed. The pathological examination identified the mass as MFH.en_US
