Prostaglandin grubu antiglokomatöz ilaç kullanımının maküler kalınlık üzerine etkisi

Abstract

Amaç: Antiglokomatöz tedavide prostaglandin grubu ilaç kullanımının maküler kalınlık üzerine etkisini araştırmak. Gereç ve Yöntem: Bu çalışmada kliniğimizde glokom tanısı ile takip edilmekte olan en az 3 yıl prostaglandin grubu antiglokomatöz ilaç kullanan 15 hastanın 30 gözü (Grup 1), en az 3 yıl diğer grup antiglokomatöz ilaçları kullanan 15 hastanın 30 gözü (Grup 2) ve benzer yaş grubunda glokomu olmayan 15 sağlıklı bireyin 30 gözü (Grup 3) kontrol grubu olarak alındı. Hastaların tümüne Optik koherens tomografi (OKT) ile foveal kalınlık, foveal volüm ve total maküler volüm analizi yapıldı ve sonuçlar karşılaştırıldı. Bulgular: Prostaglandin grubu antiglokomatöz ilaç kullanan grupta foveal kalınlık ortalama 204.673.08 ?m, prostaglandin dışı antiglokomatöz kullanan grupta 187.573.24 ?m kontrol grubunda ise 191.704.15 ?m olarak tesbit edildi. Ortalama foveal kalınlık grup 1 de, istatiksel olarak anlamlı derecede , grup 2 ve grup 3 den daha fazla saptandı (p0.05). Foveal volüm, grup 1 de 0.16570.02; grup 2 de 0.147370.02 ve grup 3 de 0.149930.03 idi ve grup 1 de foveal volüm diğer iki gruba göre istatistiksel olarak anlamlı olarak daha yüksek tesbit edildi (p0.05). Total maküler volüm ise grup 1,2 ve 3 de sırasıyla 6.789330.115, 6.564170.64 ve 6.883100.102 olup gruplar arasındaki fark istatistiksel olarak anlamlı bulunmadı. Sonuç: Uzun süre prostaglandin grubu ilaç kullanımı, foveal kalınlık ve foveal volümde artışa neden olmaktadır. Bu nedenle, kronik bir optik nöropati olan glokomda, hastaların klinik izlemlerinde foveal kalınlık ve foveal volum değişikliği de gözönüne alınmalıdır.

Purpose: To investigate the effect on macular thickness of usage of prostaglandin group eye drops for glaucoma. Materials and Methods: Thirty eyes of 15 glaucoma patients who were taking prostaglandin group antiglaucomatous drops for at least 3 years (group 1), 30 eyes of 15 glaucoma patients who were taking another group antiglaucomatous drops for at least 3 years (group 2), and 30 eyes of 15 healthy subjects of similar age (group 3) were included in this study. The maculae were evaluated by OCT, and foveal volume, foveal thickness, and total macular volume were analyzed and compared for all patients. Results: The mean foveal thickness in groups 1, 2, and 3 was 204.67±3.08 µm, 187.57±3.24 µm, and 191.70±4.15 µm, respectively. Foveal thickness was significantly greater in group 1 than in groups 2 and 3. The foveal volume in groups 1, 2, and 3 was 0.1657±0.02, 0.14737±0.02, and 0.14993±0.03, respectively. Foveal volume was significantly greater in group 1 than in groups 2 and 3 (p<0.05). Total macular volumes in groups 1, 2, and 3 were 6.78933±0.115, 6.56417±0.64, and 6.88310±0.102, respectively, and the differences between the groups were not significant (p>0.05). Conclusions: Long-term use of prostaglandin eye drops resulted in increased foveal thickness and foveal volume in glaucoma patients. Glaucoma is a chronic optic neuropathy; for this reason, foveal thickness and foveal volume should be taken into consideration in the clinical follow up of glaucoma patients.
