Post-traumatic optic nerve transection is a rare condition. It is usually complete and can be accompanied by orbital bone fractures and globe luxation. The management of such cases is quite challenging. In this report, we aimed to present the case of a 49-year-old male patient with orbital trauma, globe luxation, medial rectus muscle and optic nerve transection on the left side. Globe reduction and medial rectus muscle reposition were performed 19 days after the trauma. Five month later, the patient had no light perception in the left eye, however, he had eye movements in all directions of gaze and was satisfied with his left eye aesthetically.en_US
