Approach to in extremity and pelvic region localized metastatic bone tumors

Abstract

In this study, 28 patients with metastatic bone tumors treated in Ondokuz Mayis University Faculty of Medicine Department of Orthopaedic Surgery between March 1993 and March 1996 are evaluated. Metastatic bone defects were determined in 13 cases while 15 patients presented with pathologic bone fractures due to metastatic bone tumors. Most prevalent locations for metastatic bone disease were femur 57.2%, humerus 17.8% and pelvis 17.8%, In 39.2% of the cases metastasis originated from a lung tumor and 21.4% of cases were caused by breast tumor metastasis, but in 25% primary tumor could not be found. The mean follow up time was 21.5 (3-31) months. At the end of the follow-up period range survival was 61.6% in patients presenting with bone defects only, compared to 33.3% in those presenting with pathologic fractures. Although metastatic bone tumors suggest preterminal disease, metastasis excision and osteosynthesis with bone cement is recommended to promote life quality, early ambulation and prevent pain.
