Mooren ülserinde amniyon membran transplantasyonu

Abstract

Amaç: Tedaviye dirençli ve spontan korneal perforasyonu bulunan bir Mooren ülser vakasının taze amniyon zarı transplantasyonu ile tedavisinin sunulması. Yöntem: Bir haftadır sağ gözünde ağrı ve kızarıklık şikayetleri ile kliniğimize başvuran 69 yaşındaki erkek hastaya tipik klinik görüntüsü ile Mooren ülseri tanısı konuldu. Hastada spontan korneal perforasyon mevcuttu. Ön kamarası sığ ve görmesi 1 metreden parmak sayma seviyesinde idi. Hasta kliniğimize yatırıldı. Bulgular: Enfeksiyöz ve romatolojik olası patolojiler için yapılan laboratuar araştırmaları olumsuz sonuçlandı. Sistemik steroid, antibiyotik ile topikal siklosporin, antibiyotik ve lubrikan ajan tedavisine cevap alınamadı. Takılan bandaj kontakt lense ve verilen tedaviye rağmen inflamasyonun gerilememesi, perforasyonun genişlemesi ve iris prolapsusunun artması nedeni ile hastaya üç kat taze amniyon zarı transplantasyonu uygulandı. Transplantasyonun ardından hastanın kliniğinde rahatlama ve inflamasyonda gerileme tespit edildi. Verilmekte olan tüm antiinflamatuar tedavi ajanları azaltılarak kesildi. Bir yıl süresince takip edilen hastada ikinci bir atak tespit edilmedi. Tartışma: Amniyon zarı transplantasyonu, Mooren ülseri tedavisinde oldukça faydalı olabilmektedir. Perforasyon durumunda gözün kaybının önlenmesi amacı ile tektonik sebeplerle kullanılabilmesinin yanında, inflamasyonun kontrolünde de etkili olabilmektedir.

Amniotic Membrane Transplantation in Mooren's Ulcer Purpose: To report the treatment of a case with Mooren's ulcer and spontaneous corneal perforation by fresh amniotic membrane transplantation. Method: A male patient (69 years of age) suffering from pain and redness in his reight eye for one week was admitted and hospitalised with the diagnosis of Mooren's ulcer with typicalclinical appearance. There was spontaneous corneal peforation with iris prolapsus and shallow anterior chamber. The visual acuity was at the level of counting fingers. Results: Infectious and immunologic laboratory evaluation revealed no particular finding. Treatment was ineffective with sistemic steroids, and antibiotics, and topical cyclosporin, antibiotics, and lubricating agents. The size of corneal perforation increased with resistant inflammation in spite of use of bandage contact lens, and three layers of fresh amniotic membrane transplantation was performed for tectonic purposes. After the procedure, clinical findings improved and inflammation subsided. All the medications were stopped, and the patient was free of recurrence at the end of one-year follow-up period. Conclusion: Amniotic membrane transplantation can be quite useful in management of Mooren1 s ulcer. It can be beneficial in controlling the inflammation in addition to being an effective measure for tectonic purposes.
