Total diz protezi uygulamalarımız ve erken dönem takip sonuçları

Abstract

Total diz protezi uygulaması, diğer tedavilerin yetersiz kaldığı primer ve sekonder gonartroz olgularında iyi bir seçenek oluşturmaktadır. Bu çalışmanın amacı kliniğimizde 8 yıldır uygulanan AGC total diz protezinin erken dönem takip sonuçlarını değerlendirmektir. Ondokuzmayıs Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalı'nda Ocak 1992 ve Ocak 1998 tarihleri arasında 50 hastanın 64 dizine AGC (Anatomically Graduaded Components) total diz protezi uygulandı. Düzenli kontrolleri yapılan 40 hastanın 52 dizi değerlendirildi. Hastaların 32'si primer osteoartrit, 8'i romatoid artrit nedeniyle ameliyat edildi. Hastaların değerlendirilmelerinde "Knee Society" değerlendirme sistemi kullanıldı. Bu değerlendirmeye göre preoperatif ortalama 27.8 olan diz skoru, postoperatif ortalama 86.7 olarak saptandı. Preoperatif 40.7 olan fonksiyon skoru postoperatif 77.8 bulundu. Preoperatif 48 (%92.3) diz kötü ve 4 (%7.7) diz orta iken, postoperatif 45 (%86.5) dizde iyi ve mükemmel, 4 (%7.7) dizde orta, 3 (%5.8) dizde kötü sonuç alındı. Preoperatif ortalama 3.90 varus olarak saptanan dizilim, postoperatif ortalama 6.30 valgus olarak tesbit edildi. Preoperatif ortalama 84.30 olan hareket açıklığının, postperatif 96.20'ye yükseldiği gözlendi.Sonuç olarak; endikasyon varlığında uygun hasta seçimi ve dikkatli bir teknikle total diz protezi uygulamasından başarılı sonuçlar alınacağı kanısındayız.

Total knee arthroplasty in primary and secondary gonarthrosis is a good treatment method when other treatment modalities fails. The aim of this study is to evaluate midterm results of AGC total knee prothesis in our clinic.AGC total knee prosthesis has been applied to 64 knees of 50 patients in Orthopaedics and Traumatology Clinic of Ondokuzmayıs University Medical Faculty between January 1992-January 1998. We evaluated 52 knees of 40 patients who we were able to make follow up controls in regular intervals, thirtytwo of these patients had undergone operation due to osteoarthritis and the other 8 patients due to Rheumatoid Aril iritis. We applied bilateral total knee prosthesis in 12 patients. At the time of operation nr mean age of the patients was 64.4 (53-76) the mean follow up time was 33.7 (6-67). The patients were evaluated by using Knee Society Score System. We found out that the mean preoperative and postoperative knee scores were 27.8 and 86.7, the mean preoperative and postoperative functional scores were 40.7 and 77.8 respectively. The mean alignment was 3.90 varus before the operation we have been able to change mean alignment to 6.30 valgus after the operation. The mean total range of motion was 84.30 and it is extented 96.20 postoperatively. As a result when indicated and carried out by careful technique we thought that total knee prosthesis procedure will be succesfull.
