Fasiyal paraliziye bağlı lagoftalmi tedavisinde botulinum toksin-A ile oluşturulan koruyucu pitozis

Abstract

Amaç: Periferik fasiyal sinir paralizisine (PFP) bağlı lagoftalmi olgularında Botulinum nörotoksin-A(BoNT-A) enjeksiyonu ile oluşturulan koruyucu pitozis etkinliğinin ve güvenilirliğinin değerlendirilmesi. Gereç ve Yöntem: PFP ve lagoftalmi tanısı alan 15 hastada, cilt yolu ile levator kasına 7.5 ü BoNT-A enjeksiyonu yapılarak koruyucu pitosis oluşturuldu ve etkinliği prospektif olarak incelendi. Enjeksiyon öncesi ve sonrası rutin göz muayeneleri yapıldı, kapak aralığı, üst kapak refleks mesafesi ve levator fonksiyonu ölçüldü. Kontrollerde pitozis varlığı, derecesi, süresi ve ilaca bağlı yan etkiler değerlendirildi. Bulgular: Hastaların 8’i kadın, 7’si erkek olup yaş ortalamaları 5514.28 (22-78yıl) idi. BoNT-A enjeksiyonu ile hastaların 12’sinde (%80) ağır, 2’sinde (%13.3) orta, 1’inde (%6.7) hafif derecede pitozis elde edildi. Pitozisin ortaya çıkma zamanı ortalama 2.331.44gün, maksimum pitozis oluşma zamanı ortalama 5.732.63 gün olarak tespit edildi. Hiçbir olguda ikinci enjeksiyon gerekmedi. Pitozis ortalama 10.532.89 hafta boyunca devam etti. Pitozis gelişiminden sonra kornea bulgularında istatistiksel olarak anlamlı düzelme (p0.01) ve günlük yapay göz yaşı ihtiyacında azalma (p 0.01) tespit edildi. Hastalar ortalama 3.460.83 ay takip edildi. Takip süresince hiçbir hastada lokal ya da sistemik bir yan etki görülmedi. Sonuç: PFP bağlı lagoftalmisi olan hastalarda levator kasına BoNT-A enjeksiyonu ile elde edilen pitozis oküler yüzeyin korunmasında ve mevcut korneal komplikasyonların tedavisinde etkili ve güvenilir bir yöntemdir.

Aim: To evaluate the safety and efficacy of the protective ptosis with Botulinum neurotoxin type-A(BoNT-A) for lagophthalmos cases due to peripheral facial paralysis (PFP). Material and Methods: Protective ptosis was produced by 7.5 u Botulinum toxin type-A injection into levator muscles subcutaneously in 15 patients with PFP and lagophthalmos. Its efficacy and safety were evaluated prospectively. Routine eye examinations were performed before and after injection; interpalpebral distance, upper eye lid margin reflex distance and levator muscle function were measured. In control visits, degree and duration of ptosis, and side effects of the drug were evaluated. Results: Eight of the cases were female and 7 were male, the mean age was 55±14.28years ( 22-78 years). Ptosis was achieved with BoNT-A(BOTOX) injection which was severe in 12(80%), moderate in 2 (13.3%), mild in one (6.7%) cases. The effect of BoNT-A was started in 2.33±1.44 days and peaked in 5.73±2.63 days. No patient needed a second injection. The mean duration for ptosis was 10.53±2.89 weeks. After development of ptosis, statistically significant improvement in corneal symptoms (p<0.01) and decrease in daily artificial tear requirement p<0.01) were detected. The mean follow-up time was 3.46±0.83 months. No local or systemic side effects were observed during the follow-up time. Conclusion: BoNT-A injection for ptosis is an effective and safety method to the protect of ocular surface as well as treatment of corneal complications in lagophthalmos cases due to PFP.
